BlackTie Medical is pleased to offer our ProFormanceQA users a simple means of acquiring a month to month, single site license to
our service. We now permit users to enter a valid credit card as a payment option during your registration to use ProFormanceQA.
Please review the information provide below if you would like to use a credit card to acquire a ProFormanceQA license.
Service Description
ProFormanceQA - a 3rd party, Internet-based service for tracking quality measures in a hospital's central sterile department(s).
Users are able to create "maps" of their department's equipment and manual quality practices and then ProFormanceQA serves a
repository for all quality measurements, graphics, reports and alerts.

BlackTie Medical Sales Credit Card Payment and Refund Policy
Please review these policies carefully as they are the terms of sale that govern your credit card purchases from BlackTie Medical.
They set out your rights and obligations with respect to your purchases, including important limitations and exclusions. Your
completed registration as a user of the ProFormanceQA service from BlackTie Medical constitutes your agreement that these policies
apply to you. Please be certain you understand them before you complete your registration for ProFormanceQA.
Payment Methods
At this time we accept all major credit cards (Visa, MasterCard, American Express and Discover).
LTF Systems, Inc. reserves the right to suspend or terminate your ProFormanceQA account without notice upon rejection of any card
charges or if your card issuer (or its agent or affiliate) seeks return of payments previously made to LTF Systems, Inc. when LTF
Systems, Inc. believes you are liable for the charge. Such rights are in addition to and not in lieu of any other legal rights or
remedies available to LTF Systems, Inc.

Refunds
LTF Systems, Inc. strives to provide our registered users with the utmost satisfaction. If you are not satisfied with your use of
the ProFormanceQA system for any reason, you are welcome to request a refund. To do so, simply click the "Info" icon in the upper
right hand corner of each application page while you are logged into ProFormanceQA using your BlackTie user name. In your email
please include any specific details regarding your compliant. We will review each request within 3 business days and contact you
as required through the BlackTie application. Any single refund request will, at a minimum, equal a daily pro-rated portion of
your monthly license but no single refund request will exceed the total amount of the monthly license fee paid during the month
the refund request is submitted. All credits for approved refund requests will be issued to the credit card account that is on
file with BlackTie for your user ID.
Cancellation
Registered users of BlackTie Medical's ProFormanceQA service may cancel their contract at anytime by clicking the "Info" icon in
the upper right hand corner of each application page. In the subject line of your email simply put "Request for Cancellation".
We will contact you via phone at the number(s) on file in your user profile to confirm your request. License fees already paid
for the month of cancellation will be pro-rated for the remaining days in the month and refunded to the credit card on file.
